Vitamin D has been misnamed, misunderstood, and chronically underestimated by conventional medicine. Here’s what it actually does — and why getting it right might be one of the most important things you do for your health.
Nonsense filter alert: Vitamin D is not a vitamin. It’s a prohormone, a precursor compound your body converts into an active hormone that then goes to work across virtually every system you have.
The vitamin label was assigned before anyone understood what it actually did. And that mislabeling has had real consequences for how seriously conventional medicine has treated it, which is to say, not seriously enough.

The Hormonal Truth They Glossed Over
When vitamin D enters your body — from sunlight, food, or supplements — it isn’t active yet. It goes through a two-step conversion: first in your liver, then in your kidneys, before finally becoming the active hormonal form your cells can actually use. That active form, calcitriol, is what does the work. And that work is enormous.
We’re talking about a compound that:
- Regulates immune response
- Supports cardiovascular function
- Modulates your endocrine system
- Drives the synthesis of serotonin and dopamine — your mood and motivation neurotransmitters
- Acts as a neuroprotective agent, reducing neuroinflammation and supporting brain plasticity
- Has been linked by research to depression, anxiety, ADHD, chronic pain, and cancer risk when levels run low
- Regulates calcium absorption, which is critical for maintaining bone density and strength and preventing osteoporosis. It also aids in bone remodeling and reduces the risk of fractures by improving muscle function
This is a hormone precursor that your body uses to run hundreds of biological processes. The fact that the RDA sits at a number designed to prevent rickets — a deficiency disease — and not to support optimal function tells you everything about how Western medicine has approached this.
The official RDA is 600 IU per day for adults under 70, and 800 IU for those over 70 — numbers explicitly set to maintain bone health, not systemic hormonal function. Here’s what’s telling: even the Endocrine Society and Mayo Clinic quietly acknowledge that 1,000–2,000 IU daily is more appropriate for most adults. They’re admitting the RDA is too low. They just haven’t made it loud yet. Functional practitioners go further, recommending whatever dose gets your blood into the 60–80 ng/mL optimal range — which is a completely different conversation.
What “Low” Really Means
Conventional labs flag vitamin D adequacy at 20 – 35 ng/mL. Anything above that, and most doctors will tell you you’re fine. Below 12 ng/mL is where they formally call it deficient. That’s a remarkably low bar — and it was set based on bone health data, not brain health, immune function, or hormonal activity.
Most integrative and functional medicine practitioners won’t accept anything under 50 ng/mL as optimal — and many aim for 60–80 ng/mL. Think about that gap: you can be sitting at 22 ng/mL, get a normal on your lab report, and be running at less than half the level that functional medicine considers the floor for real health.
To put the RDA in context: a single session of midday sun exposure on bare skin can generate 10,000 IU or more. The 600 IU daily recommendation — built around minimal sun exposure and bone disease prevention — isn’t even close to what your body is capable of producing naturally. And yet that’s the number your doctor is working from. Even Mayo Clinic now states that 1,000–2,000 IU daily is generally safe and more clinically appropriate for most adults. The Endocrine Society echoes this. The official position is being quietly revised. It’s just not making it into most exam rooms.
Not All Vitamin D Is the Same
This is where supplementation gets more nuanced than most people realize. There are two forms you’ll encounter:
Vitamin D2 (ergocalciferol): The plant-derived form, often used in fortified foods. It’s less bioavailable and converts to the active form less efficiently in the body. This is often what you’re prescribed if a doctor does decide to supplement you. I’m not a fan.
Vitamin D3 (cholecalciferol): This is the form your skin synthesizes from sunlight. It’s more potent, raises blood levels more effectively, and is better retained in the body. This is the one you want.
But here’s the part that gets overlooked even when people are buying the right form: vitamin D is fat-soluble. That means it requires fat to be properly absorbed. A dry tablet or capsule sitting in your stomach with no fat present is not being absorbed anywhere near as efficiently as it could be.
The only properly delivered vitamin D supplements are gel caps — softgels that contain D3 suspended in oil — or liquid drops taken with a meal that contains fat. Dry tablets and hard-pressed capsules of D3 are significantly less bioavailable. This is not a minor distinction.
Look for D3 in olive oil, avocado oil, or MCT oil as the carrier. Liquid drops are another excellent option — they’re easy to dose precisely and absorb well when taken with food. If your supplement isn’t in a lipid medium, you’re leaving a lot on the table.
The Cofactors That Make It Actually Work
Even when you’re taking the right form in the right delivery vehicle, vitamin D doesn’t operate in isolation. There are several nutrients it works alongside that are critical to its function — and most people are missing at least one of them.
Vitamin K2: This is the most important cofactor and the one most often left out of the conversation. K2 directs calcium to your bones and teeth and away from your arteries. When you’re raising vitamin D levels, calcium absorption goes up. Without K2 doing its traffic-directing job, that calcium can deposit in soft tissue. If you’re taking D3, you should be taking K2 with it — specifically MK-7, the longer-acting form. This is non-negotiable in my view.
Magnesium: Magnesium is required for multiple steps in the vitamin D conversion process. A significant portion of the population is deficient in magnesium, which means their vitamin D conversion is compromised at the enzymatic level before it even reaches the receptor. Glycinate or malate forms are generally easiest on the stomach.
Zinc and Boron: Both support vitamin D receptor function and overall mineral metabolism. They’re not the headline act, but they matter, especially if you’re dealing with absorption issues.
D3 + K2 + Magnesium is the foundation. If you’re not taking all three together, you’re running an incomplete protocol.
The VDR — When the Lock Won’t Open
Even if you’re doing everything right — right form, right delivery, right cofactors — your cells still have to be able to respond to vitamin D when it arrives. That happens through the vitamin D receptor, or VDR. Think of it as a lock, and active vitamin D as the key. When it works, a cascade of biological activity follows across immune response, inflammation, serotonin synthesis, and more.
Some people have genetic variants in their VDR that compromise that binding. The key shows up, but the door doesn’t open as wide. Sound familiar? Have you ever taken 5,000 IU a day for months and had your level barely budge? That’s not in your head. That’s biology.
VDR variants are not rare, and their effects are real — research has found striking correlations between specific VDR genotypes and severe COVID-19 outcomes, which tells you something about how fundamental this receptor is to immune regulation. A clinical trial of nearly 1,800 people confirmed it: the same dose of D3 produces dramatically different results depending on your genetic profile. Same pill. Very different outcomes.
If your numbers won’t move despite a solid protocol, the answer probably isn’t to take more. It’s to understand what’s blocking it.

The out-of-pocket cost for a 25-OH vitamin D blood test is typically between $30 and $60 through direct-access labs. That’s a cheap piece of data. Without it, you’re guessing. You don’t know if your level is 18 ng/mL or 72 ng/mL. You don’t know if three months of supplementation is working. You can’t make an informed decision about your dose.
Insurance companies will often only cover vitamin D testing if you have a documented deficiency or a specific condition that warrants monitoring. So if you’re testing proactively — which is exactly what you should be doing — you may get denied. Pay the $40 anyway. This is too fundamental to guess at.
And if your levels genuinely won’t respond despite doing everything right — D3 in oil, cofactors, consistent protocol — that’s when it’s worth going deeper to understand whether a VDR variant or a conversion issue might be at the root of it.
Knowing your number is not optional. You cannot optimize what you don’t measure. A $40 – $50 test once a year is cheap insurance for one of the most foundational hormones in your body.
The Bottom Line
Knowing that Vitamin D is not is more than just a vitamin, but an important part of regulating and maintaining a strong immune system, cardiovascular system, brain chemistry, mood, inflammation levels, sex hormones, and hundreds of genes. The government’s recommended intake was built to prevent a bone disease, not to support the kind of vibrant, functioning health we’re actually after.
If you’re supplementing, make it D3 in a gel cap or oil-based drops. Take it with a meal that has fat in it. Pair it with K2 (MK-7) and magnesium. Get your blood levels tested — even if you pay out of pocket. And if your numbers chronically underrespond, consider that your genetics might be part of the story.
This is one area where doing things conventionally is genuinely leaving health on the table. The research is there. The tools are accessible. And now you know that truth about the secret life of vitamin D.
